When marking assessment if you give feedback in the trainer feedback box then click straight on the not-satisfactory button about every second or thrird time as it is auto saving it wipes the comment you had just written and there is no way to get it back.

This is usually caused by your change not being saved and then clicking an outcome button causes an update to the field. Try clicking out of the box before outcoming a result if this continues.
